You addressed the mold. You treated Lyme. You left the water-damaged building. So why are you still sick? This is a question I hear from patients who have done many of the "right" things but still struggle with fatigue, brain fog, pain, dizziness, sensitivities, sleep problems, or other persistent symptoms.
The answer is often that the original exposure was only part of the story. I am less interested in finding another name for your symptoms. I want to know what is keeping your body from getting back into balance. That means looking upstream at the root causes instead of continuing to chase the downstream mess.
The Trigger Is Not Always the Whole Story
Mold exposure, water-damaged buildings, and vector-borne infections can all be significant health stressors. But the trigger that started an illness may not be the only thing keeping it going.
Think about knocking over the first domino in a long line. Picking up that first domino does not automatically put all the others back in place.
The same idea can apply to chronic illness. By the time we identify the original trigger, several other problems may have developed. These can include:
- Immune system imbalance
- Chronic inflammation
- Allergies and sensitivities
- Autoimmune activity
- Secondary infections or microbial overgrowth
- Hormonal disruption
- Nervous system dysregulation
- Problems tolerating foods, supplements, or treatments
I often refer to these problems as the "downstream mess." The symptoms are absolutely real, but they are happening downstream from something else.
Immune Dysfunction May Be the Missing Middle
One of the most important pieces of this puzzle is the immune system. At Restorative Medicine Center, immune imbalance sits at the center of our Root Cause Triad.
When immune function becomes disrupted, the effects can look very different from one patient to another. Some people develop obvious immune problems. Others collect seemingly unrelated symptoms and diagnoses over time.
Signs that make me think more carefully about immune dysfunction include:
- Multiple allergies or sensitivities
- Autoimmune conditions
- Recurrent or persistent infections
- Chronic inflammatory symptoms
- Fatigue and post-exertional symptoms
- Brain fog or cognitive changes
- Unexplained pain
- Multiple diagnoses that do not explain the entire picture
This is also where CIRS, or Chronic Inflammatory Response Syndrome, becomes part of the discussion. CIRS describes a pattern of chronic inflammatory and immune dysfunction associated with biotoxin illness.
What Can CIRS Biomarkers Tell Us?
There is no single blood test that explains a complex chronic illness. Instead, certain biomarkers can provide clues about what the immune system is doing.
Some of the biomarkers I use include:
- C4a
- TGF-beta 1
- MMP-9
- ECP
- VEGF
- Vitamin D ratio
- Immunoglobulin levels
- ANA and other autoantibodies
- hsCRP
Mold, Microbes, and the Environment Can Overlap
Water Damage and Mold Exposure
Water-damaged environments can expose people to mold and related biotoxins. If that exposure continues, it can become difficult to make progress.
That is why indoor air quality matters. We routinely encourage patients to investigate mold problems when appropriate. Improving the environment may be an important part of reducing ongoing exposure.
Lyme Is Not the Only Microbe We Consider
The same problem occurs when everything gets blamed on Lyme disease. Borrelia can matter, but it is not the only vector-borne organism worth considering.
Depending on the patient's history, organisms may include:
- Borrelia
- Bartonella
- Babesia
- Other bacterial organisms
- Fungal organisms such as Candida or Aspergillus
- Secondary microbes that become harder to control when immune function is impaired
The Root Cause Triad: Why We Look Beyond One Diagnosis
Over the years, my approach has evolved into what I call the Root Cause Triad. It gives us a framework for organizing complicated cases. Instead of asking which diagnosis explains everything, we investigate three interconnected areas.
Microbes
Microbes include bacteria, viruses, fungi, and parasites. I think about them as primary and secondary issues.
Some microbes may contribute directly to immune dysfunction. Others may persist because the immune system is already struggling.
Toxins and Biotoxins
We also evaluate relevant toxin and biotoxin exposures. Environmental mold is one possible source.
Microbes themselves may also contribute to the body's biotoxin burden. That creates an important connection between the microbial and toxin sides of the triad.
The Stress Response
The third piece is often overlooked. You can address microbes and environmental exposures while your body remains stuck in fight or flight.
The amygdala constantly evaluates whether your environment is safe or threatening. Chronic illness itself can contribute to hypervigilance.
When the sympathetic nervous system remains activated, the body can stay in a prolonged danger response. In my treatment paradigm, this can interfere with healing.
"I Treated It" and "My Body Recovered" Are Different Things
This is one of the biggest concepts I want patients to understand.
Completing a treatment does not necessarily mean every downstream effect has resolved. Likewise, removing an exposure does not mean immune function immediately returns to normal.
Recovery may require addressing several areas at once, including:
- Ongoing environmental exposure
- Relevant microbial issues
- Immune and inflammatory dysfunction
- Sleep quality
- Hydration and nutrition
- Appropriate movement
- Nervous system regulation
- Treatment tolerance
Foundational habits matter here. Clean air, adequate hydration, restorative sleep, nourishing food, movement, and stress reduction support the larger treatment strategy.
They are not substitutes for identifying root causes. They help create an environment where the body has a better opportunity to recover.
My general treatment philosophy is simple: stay calm and cover the root cause issues at a rate you can handle. Then keep doing that long enough for the downstream mess to improve.
More Treatment Is Not Always Better Treatment
When you have been sick for years, there is an understandable temptation to throw everything at the problem. Five new supplements. Multiple antimicrobials. A new diet. Detox therapies. A sauna protocol. Several other changes at the same time. Then you feel terrible. Or maybe you feel better. Either way, we have a problem.
Your Response Is Valuable Data
I want to know two things about each intervention:
- Is it tolerated? If so, what is the best dose?
- Is it beneficial? If so, how?
That is why I generally recommend separating new medications or supplements when possible. Patients should also track changes in symptoms.
Feeling worse after treatment can have several explanations. It should not automatically be labeled a Herxheimer reaction and pushed through.
Track the Cause and Effect
A simple calendar can become an incredibly useful clinical tool.
Track your most important symptoms along with:
- Medication changes
- Supplement changes
- Treatments
- Diet changes
- Activity levels
- Significant stress
- New environmental exposures
Getting Unstuck Means Looking at the Whole Pattern
If you have been sick for a long time, you may already have a thick medical chart. More information is not always what is missing.
We need the right information.
A deeper evaluation may look at:
- When symptoms actually started
- What happened before they started
- Previous water damage or mold exposure
- Possible vector-borne exposures
- Infection history
- Immune and inflammatory patterns
- Previous treatment responses
- Treatments that caused significant flares
- Current environmental exposures
- Changes in symptoms over time
Testing should help us answer clinical questions. Ordering every test available is not the same thing as digging deeper.
Labs provide clues, but they are only part of the picture. I also want to know what happens when we make a change.
Healing Means Changing What Is Driving the Illness
Complex chronic illness is rarely tidy. Mold, water damage, vector-borne infections, immune dysfunction, and the stress response can overlap. The factor that started the problem may not be the only factor keeping you sick today.
That is why I keep coming back to the root causes. Instead of asking only whether you "still have Lyme" or "still have mold illness," we ask a better question. What is still driving immune dysfunction now, and what can we reasonably change?
